Use of this procedure may vary with custom in various areas of the state and with individual financial institutions’ practices. In situations in which the decedent had a will which is not being offered for formal admission to probate, a copy of the will may be required to be exhibited. Form 32 may be used with this form. An unsworn declaration pursuant to K.S.A. 53-601 may be used as an alternative to this affidavit form.
